(As of 5/25/22) We are currently in OUTBREAK mode.
We are currently in OUTBREAK mode at Meridian Meadows Transitional Care!
We will not be restricting visitation, however, we will require masks to be worn in the facility at all times while we remain in outbreak protocol.
We are always following recommended CDC and State COVID-19 guidelines. We will continue to encouraged employees to be mindful of their surroundings while out and about outside of the workplace.
We strive to be diligent in doing all we can as staff, family members, and as a community to prevent the spread of COVID-19. As a whole, we can help protect the vulnerable population that we take care of here at Meridian Meadows Transitional Care.
